What is Palliative Care at Home?

At Stable Home Care, we understand that palliative care is a deeply personal journey. Our palliative care at home service is designed to support older adults and their families through the the whole journey from accessing home support to their end of life journey with empathy, respect and practical support. This allows our older adults to remain in the comfort of their own home, surrounded by familiar people and their loved ones.

Whether you’re caring for a loved one or planning ahead for yourself, we walk alongside you to provide personalised care that focuses on comfort, dignity and quality of life.

That around 70% of Australians say they would prefer to die at home, only about 14% actually do. Most people spend their final days in hospitals or residential aged care. With the right support in place, it is possible for more older adults to remain at home, surrounded by the people and comforts they know and love. How We Support You at Home

Our palliative care support is flexible, responsive and centred around what matters most to you and your family. We understand that needs can change quickly, which is why we work in close partnership with your GP, local palliative care services and hospitals to ensure consistent, high-quality support – day or night.

Personal Care


We support daily routines with sensitivity and respect. This includes help with bathing, dressing, grooming, mobility, toileting, and continence care – all while preserving your comfort, independence and dignity.

    Palliative Care Nurse


    A Palliative Care Registered Nurses play a vital role in supporting individuals with a life-limiting illness to remain comfortable and well cared for at home. At Stable Home Care, our experienced nurses provide expert clinical support such as managing pain, administering medications, monitoring symptoms, and responding to changes in health. Just as importantly, they offer calm, compassionate presence, taking the time to listen, answer questions, and guide families through what can be an overwhelming time. 
    24/7 Support


    When care needs increase, we can arrange overnight or 24/7 support for continuous reassurance and safety. This is particularly helpful during periods of instability or towards end of life, when being close by matters most.

    Care Coordination


    Our nurses work closely with your GP, palliative care specialists, and family to ensure your care is well coordinated, consistent and aligned with your personal values and goals. We take the lead in bringing everyone together so that communication is clear, your needs are understood and support is seamless across all aspects of care.

    Respite for Families or Carers


    Caring for a loved one can be physically and emotionally draining. We provide in-home respite so that family carers can rest, recharge, or take care of other commitments, knowing their loved one is in safe, compassionate hands.
    Assistive technology


    We can help you access assistive technology that supports comfort, safety and independence at home during end of life care. This may include hospital beds, pressure-relieving mattresses, mobility aids, personal alarms or other equipment. Our team will assess your needs and work with suppliers, your care team and funding options to make sure the right equipment is in place when you need it most.

    How Is Palliative Care at Home Funded?

    Support at Home - End of Life Funding

    Do you have a Support at Home Package? The Government has launched a new End-of-life funding program to support older adults to die at home. Speak to one of our Aged Care Experts about about to access this program. 

    NDIS

    We provides services to NDIS participants. If you are a participant that has been given a life limiting diagnoses and would like to die at home. Please speak to one of our Care Experts about your options.

    DVA

    We are an Approved DVA Community Nursing Provider and are available to take referrals for Palliative Care at Home through our Community Nursing Program. Contact us below.

    Self Funded

    Our services can be paid privately with rebates available for selected services that are covered under your private health insurance. For more information on rebates specific to your situation, please contact your private health insurer.

    Stable Home Care provides personalised in-home care services across the Gold Coast and Tweed Heads region. We provide expert guidance for managing chronic conditions, provide support with personal care, and Nursing Supports. Our Registered Nurses work in collaboration with your GP and specialists to deliver holistic, tailored approach, promoting comfort and independence.     A Palliative Care Nurse will visit you in your home and complete an assessment and organise any ongoing supports tailored around your goals and care needs.

    Feel Supported at Home 

    ​You will have access to a dedicated Care Manager to support you and address any questions you may have. Sit back and enjoy your services.
